The form is located under the "Inside Atheltics" tab on our website or you can click on the link provided below. After filling out the form, you can email it to richanna.lindo@sunycgcc.edu or mail it to Richanna Lindo, Director of Athletics, Columbia-Greene CC, 4400 State Route 23 Hudson, N.Y. 12534

You can email or mail any additional information that may help the Hall of Fame Committee make their decision. Please provide as much supporting information as possible.
